Financial/Insurance

We are dedicated to providing the best possible care to you and your family. Your complete understanding of your financial responsibilities is an essential element of your child’s care and treatment. Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Payment is required at the time of service. Cash, checks, Visa, MasterCard, American Express and Discover cards are acceptable forms of payment in our office. We also offer several financing options if you need extended payments, it only takes a few minutes to complete an application.

Patients on insurance plans are asked to pay their co-payment, co-insurance, deductibles or any other non-covered services at the time of their visit. As a courtesy, we will file your primary insurance claim for you. Secondary insurance is only billed for you if we are a contracted provider. You will be billed directly if your insurance company does not cover a service. If your insurance does not pay the doctor in a reasonable length of time (within 30 days), we will have to look to you for payment. We encourage you to call your insurance company and make sure they have gotten the claims and are going to pay them in a timely manner.

Non-Covered Services
Patients are expected to know the benefits provided by their insurance company. Please contact your insurance company to review your benefits for dental services. Example: Do you have to go to a specific doctor in order to receive benefits? Do you have in- and/or out-of-network benefits? We may be out-of-network. Is a referral necessary because we are a specialist? Is your plan simply a discount plan? What is the discount? Do they pay benefits by their reasonable and customary fee (UCR fee)? Are there any waiting periods and limitations to any services?

Change in Insurance Plans
You are expected to notify our office if your insurance coverage changes. Our office will periodically ask you to update your records. You will be expected to provide full and complete information to our office in order for us to bill the correct insurance company for you.
