Office Policies

Appointments
Your scheduled appointment time has been reserved specifically for you. Our office requires a 24-hour notification if you need to cancel or reschedule an appointment. Failure to contact our office in advance prohibits our providers from treating other patients who are in need of care and creates a hardship on the practice. A fee will be billed if you fail to show or do not cancel in advance.

Insurance
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill your insurance company for services and allow them 30 days to render payment. After this time, you are responsible for the full balance. We encourage you to call your insurance company and make sure they have received the claims and have paid accordingly.

Dr. Umar Imtiaz DDS MS

Dr. Imtiaz received his doctoral degree from Columbia University before completing an internship and specialized residency in pediatric dentistry at the Children's Hospital Montefiore Medical Center in New York City. He is a member of several professional and medical organizations, including the American Dental Association, American Academy of Pediatric Dentistry, Academy of General Dentistry and the Arizona Dental Association. Dr. Imtiaz has completed over 4000 oral conscious sedations on children of all ages. He is also an 1303 Sedation Permit examiner for the Arizona State Board of Dental Examiners. Aside from his professional obligations, Dr. Imtiaz also enjoys Red Sox baseball, travel, barbecues, Formula 1 racing, and spending time with his family.
